Joan and Richard Hewett seem not to understand sadness or irony in their photo essay ''On Camera,'' the story of the life and times of Philip Waller, child actor. Of course, the Hewetts, authors of ''Motorcycle on Patrol: The Story of a Highway Officer,'' don't have Miss Gish's perspective: her 105th movie, ''The Whales of August,'' opened on Oct. 14, which was probably her 91st birthday (there is some question about her exact age because she never had a birth certificate).
